The medium pouch is much longer than the small version and may be too tall for some daypacks. The Gatekeeper hooks attach directly to compatible Mystery Ranch packs and can provide quick access to items such as first-aid supplies. The reviewer says the small size works inside smaller travel backpacks, most daypacks, and potentially smaller slings.
The reviewer explicitly states that the pouch is not waterproof, though it keeps adventure gear relatively dry during inclement weather. The reviewer appreciates the open interior because it supports varied uses instead of restricting the pouch to one organizational purpose.
